Company DescriptionNorth Star Special Services provides residential services for individuals that have the following types of disabilities: Behavioral Problems, Blindness/ Severe Visual impairment, Mental Retardation, Cerebral Palsy, Orthopedic Impairment, Epilepsy, Spinal Cord Injury, other Neurological Impairment, and other undetermined disabilities. Job DescriptionSupervises the daily routines of the residence in accordance with applicable standards and regulations. This includes, but is not limited to, environmental, fiscal and medical requirements, individuals' treatment regimens and ensuring that appropriate written documentation is completed as scheduledResponds to crisis situations expediently and provides the appropriate input and support needed to resolve the situationB. Specific1. Provides support and assistance to the IP Coordinator in identifying clients’ values and needs 2. Demonstrates knowledge of assigned individuals’ goals, Goal Plans, supplemental procedures and Behavior Plans and assures they are implemented as written, including training the Residential Counselors in the implementation of the IP, ongoing monitoring of implementation and re-training of Residential Counselors as needed3. Provides supervision and supports needed to assure that each residence presents a clean, neat and safe environment; this includes, but is not limited to, assuring that the residence will pass safety and environmental inspections4. Assures each Residential Counselor provides a current telephone number by which he/she may be reached expediently 5. Provides each Residential Counselor with the support, direction, supervision, demonstration and follow- up needed to insure that their assigned clients acquire appropriate community integration skills. This includes, but is not limited to, assuring the clients’ participation in residential and community based leisure and social activities, assuring access to community amenities, encouraging communication with family and friends, encouraging and assisting individuals in developing and utilizing effective self-determination and self-advocacy skills and encouraging, instructing and supporting individuals in developing the functional life skills and assuming the responsibilities required for successful integration into the community6. Provides supervision and support to Residential Counselors during any transitions such as Emergency room visits, residential moves, etc.7. Assures transportation is available to clients as required by programmatic needs, including assuring that clients utilize community based transportation and assuring they receive training in use of such transportation8. Insures that the Residential Counselors are familiar with the daily proactive schedule, assures the schedule is visible to clients and assures the schedule is followed9. Assures that individuals participate in all required/scheduled meetings, appointments and daily activities10. Assures that Just Community Meetings are conducted at least once weekly and completes written documentation of these meetings11. Attends and participates in annual and interim Interdisciplinary Team Meetings as directed and attends and participates in all in-service training as scheduled12. Attends other meetings as scheduled or as directed by the Operation Coordinator 13. Immediately reports incidents according to Agency policy for Incident Reporting and Investigation Procedures. Is available to participate in the investigation of incidents as requested14. Provides direction and support to staff during staff emergencies such as callouts and ensures the unit is properly covered to meet the needs of the individuals as stated in the IP. 15. Provides Residential Counselors with consistent supervision and support throughout their shifts; this includes, but is not limited to, providing client supervision in the residence until correct staffing ratios are attained and adjusts his/her schedule to meet the needs of the unit. 16. Assures that appropriate follow up/corrective action is taken in response to problems or deficiencies identified during Program Observations and records Audits as appropriate17. Assures promotion of clients’ values and rights through continuous monitoring of quality of service. Identifies barriers to the delivery of services specified in the Individual Plan and provides immediate follow up with the residential staff; if necessary, contacts appropriate management staff for additional support and follow-up18. Assures staffing ratios are in compliance with those designated in clients’ Individual Plans and assures compliance with Agency Policies and U.S Department of Labor Wage and Hour laws19. Maintains Medical and Program Records to assure records are current, orderly and contain all required documentation20. As directed by the Operation Coordinator, provides written documentation of individuals’ performance, status and progress as related to their Individual Plans21. As prescribed by the Operation Manager, completes evaluations of Residential Counselors and submits them to the Residential Director for review and approval22. Provides Instruction and leadership to the Residential Counselor even during difficult situations, to include but not limited to, conflict resolution, OHCQ/DDA surveys, Nursing reviews, or internal audits. Remaining calm and composed during difficult situations to offer staff support and leadership needed during these times. 22. Performs other duties as directed by the Operation Coordinator or his/her designee.QualificationsAbility to interact positively with individuals, co-workers, community residents, family, advocates and Agency staff (including, but not limited to, Residential Counselors, Residential Directors, Medical Staff and Psychological Services Staff)Ability to provide direction and leadership to subordinate staffAbility to provide instruction, training and support to developmentally disabled individuals Additional InformationThe ideal candidate would have the following: A Bachelor’s Degree from an accredited college or university in mental health, social services, business or a related human services field.orAt least two years of equivalent experience providing professional services to the developmentally disabled. andSuccessful completion of all DDA and agency required training and possession of a valid Driver’s License and reliable transportation.
